Create a student support plan

Focus support on evidence

A student support plan turns a specific observation into a focused goal, practical strategies, and a review point instead of leaving concerns as an unstructured note.

Start with observed facts

Describe what was seen or heard in a defined context, keeping those observations separate from interpretations about ability, intention, or diagnosis.

Set a workable goal

Choose one attainable support goal that can be recognised in classroom practice, rather than combining several broad needs into a single plan.

Specify support actions

State who will provide each strategy, when it will happen, and what evidence will show whether the approach is helping the learner.

Review with care

Check that wording is respectful, accurate, and suitable for the intended audience before sharing this sensitive plan with the learner, family, or support team.
